Do you have a child between 12 and 17 years old?
We invite teens to participate in a 5-minute survey about moods and well-being.Teens will complete a 5-minute online survey about their moods and well-being.
Your child will be entered into a raffle with a 1 in 50 chance to win a $100 gift card.
